We are pleased to welcome you to our dental practice, and we are happy that you chose us for your dental care. Your first visit will include an X-rays and an exam of the teeth and gums. We offer a comprehensive range of dental services designed to improve and maintain excellent oral health. 

Please bring your dental insurance information (card, employer, spouse, if primary holder). If you had a recent set of x-rays taken, you can request a copy from your previous dentist. 

If you are a new patient, please download and print the New Patient forms below and bring or email the completed forms to the office.  

If you are a patient of record or if there any changes in your personal information (Insurance, Address, Phone number) and/or if it's been more than 1 year since your previous visit please complete the pages that pertain to those changes and bring to the office or email prior to your visit.

If you are a patient of record and wish to have your x-rays sent to another office, please sign the Release form and return to the office.
